II. ELIGIBILITY
Contest is open to all legal U.S. residents age 18 years and older as of entry date. Employees of Direct Source and members of their immediate families or households are not eligible to enter. No purchase necessary.
III. HOW AND WHEN TO ENTER
To enter, go to http://www.shopdirectsource.com or print your name, address, telephone number, age and the words “DIRECT SOURCE SWEEPSTAKES” on a 3” x 5” card and mail to: Direct Source, P.O. Box 270530, Flower Mound, TX 75027. Limit one entry per person. All entries must be postmarked by May 23, 2008 and received by May 30, 2008. Sponsor assumes no responsibility for lost, late, damaged, misdirected, illegible, incomplete, incorrect or postage-due mail entries, errors due to computer malfunctions, software errors, transmission problems or interruptions in operations, lost or misdirected email, all of which cause entries to be void. In the event of a dispute regarding the identity of the person submitting an electronic entry, the entry will be deemed to be submitted by the “authorized account holder,” person in whose name the e-mail account is registered, provided that person meets all eligibility criteria set forth in these Official Rules. Any attempt to defraud or misrepresent the age, name or address by an entrant may be prosecuted to the fullest extent permitted by law. All entries become the property of the Sponsor and will not be returned.
IV. HOW AND WHEN WINNERS WILL BE SELECTED AND NOTIFIED
Odds of winning depend on number of valid entries received. Winners will be determined in a random drawing on or about June 1, 2008. The winners will be notified by email and US mail immediately and will be required to sign an affidavit of eligibility and liability release within 14 days of notification or prize will be forfeited and may be awarded to another winner. If winner cannot be located or does not respond within 14 days, prize will automatically be forfeited and an alternate winner will be selected. If prize notification letter or prize is returned as undeliverable, the corresponding prize will be awarded to an alternate winner. Except where prohibited, acceptance of prize constitutes winner's consent to use his/her name, likeness and biographical data for advertising and promotional purposes without additional compensation.
V. PRIZES
One (1) grand prize winner will receive $1,000.00 USD. Ten (10) first-place winners will receive one (1) iPod® nano (approximate retail value $149.00 each). Sponsor reserves the right to substitutes prizes described above at sponsor’s discretion for a prize of equal or greater value. Prizes are non-transferable. All federal, state and local taxes on the prize are winner's sole responsibility. Grand prize winner will receive an IRS Form 1099. By accepting the prize, winner waives the right to assert as a cost of winning said prize, any and all costs of redemption or travel to redeem said prize and any and all liability that might arise from redeeming or seeking said prize.
VI. RELEASES
Winner agrees to release J.W. Direct Source, Inc. and its respective employees, officers, directors, agents, representatives, parent companies, affiliates, suppliers, distributors, advertising, promotional and judging agencies from any and all liabilities, claims or actions of any kind whatsoever for injuries, damages or losses to persons or property which may be sustained in connection with receipt, ownership or use of any prize. It is the sole responsibility of the entrant to notify the sponsor in writing if the entrant changes his/her e-mail or home address prior to the selection of winner.
VII. WINNERS LIST
Winners’ names will be posted on http://www.shopdirectsource.com for a period of 30 days following the drawing.
